- After receiving your Certification of Eligibility, send a copy to the Veterans' Certifying Services located in the OSU Registrar's Office in Corvallis.
If benefits were received at a different institution prior to admission at Oregon State University:
- Fill out a VA Form 22-1995 Change of Place of Training Form/from VA website. Send a copy to the Veterans' Certifying Official, located in the OSU Registrar's Office.
- Or fill out and submit to the Registrars’ VA Form 22-5495 if your dependent or spouse are receiving benefits

Step 3: Make sure you have requested your Academic Advisor to verify your upcoming term courses from COCC to the OSU Veterans Certification Services.
Oregon State University – Cascades Students (who are taking any COCC courses)
- Using benefits to cover courses taken simultaneously at both OSU and COCC. You are receiving benefits through OSU; we are your parent school. COCC needs approval from the Veterans' Certifying Official at OSU Corvallis allowing them to certify your courses at their institution.
- Contact your OSU-Cascades advisor to get approval for the courses you plan to take in the following term. Students must email their OSU-Cascades advisor your COCC courses each term as soon as you register.
- Your OSU-Cascades advisor will e-mail his or her approval to the Veterans' Certifying Services at OSU Corvallis.
- The email must include a statement that courses being taken apply to a degree program offered at OSU. The statement must include the course number and name of the class.
- The Veterans' Certifying Services in OSU Corvallis will send a "Parent School" Letter of approval to COCC.
- COCC will then be able to certify you to the VA for the specified courses.
- The Veterans' Certifying Services at OSU Corvallis will then issue enrollment certification documents to the appropriate VA Regional office in order to monitor the student's satisfactory academic progress.
Step 4: Utilize your VA Education Benefits
Before making a potentially irrevocable decision on which benefit to use, consult with the Veteran Certifying Services first.
